Responsibilities
- Provide LPN-level nursing care to home care clients according to the plan of care.
- Monitor vital signs, assess changes in patient condition, and document accurately in the chart.
- Administer prescribed medications and treatments under supervision; perform wound care and related assessments with proficiency.
- Collaborate with supervising RN and/or physician to adjust care plans as needed.
- Educate patients and families on disease management, medication safety, and care routines; reinforce infection control and safety practices.
- Ensure quality documentation, maintain a safe home environment, and respond promptly to clinical changes.
Requirements
- Active Licensed Practical Nurse (LPN) license in Pennsylvania
- Graduation from an approved practical nursing program
- Minimum of 2 years of home care experience
- Proficiency in wound care
- Authorized to cover assignments in the 19104 and 19139 ZIP code areas
- Ability to travel within the local service area and maintain a consistent 8:00 am–4:30 pm schedule
