This travel assignment is for a post-acute skilled nursing rehab unit within a long-term care setting. The LPN will provide direct patient care to adults 60+ on a rotating schedule with occasional floating to other unit needs. The position uses EPIC for documentation and requires prior travel experience and EPIC proficiency.

Responsibilities
- Deliver direct nursing care to post-acute residents in a skilled nursing rehab environment, including medication administration and IV therapy as ordered.
- Monitor vital signs, assess changes in patient condition, document in EPIC, and respond to clinical alarms and call bells.
- Perform phlebotomy, EKGs, transports, starting IVs, nebulizer to inhaler administration, and other unit-specific procedures as trained.
- Collaborate with the interdisciplinary team to develop and implement care plans, participate in patient rounds, and ensure continuity of care during transitions between therapies.
- Maintain patient safety and comfort, assist with activities of daily living as needed, and adhere to dementia-capable care protocols when applicable.
- Adhere to holiday requirements and respond to unit needs, including occasional on-call or extended shifts as per contract.
Requirements
- Active Vermont LPN license
- BLS certification
- EPIC EMR experience within the last 2 years (nonnegotiable)
- Minimum 1 year of clinical experience in a relevant setting (long-term care or post-acute)
- Prior travel experience required
- IV therapy certification preferred; dementia-capable care certification preferred
- Ability to float to additional units as needed and work 12-hour night shifts
- Must be able to work all holidays as required
