Responsibilities
- Provide direct nursing care to post-acute skilled nursing residents, including medication administration, wound care, and care coordination per physician orders.
- Monitor patient status with routine assessments, vital signs, and documentation in the EPIC electronic health record; communicate changes to the care team.
- Perform IV therapy where certified, plus phlebotomy and EKGs as trained; support accuracy of medication administration and specimen collection.
- Assist residents with activities of daily living and rehabilitation goals, incorporating dementia-capable care strategies as appropriate.
- Maintain a safe, clean, and compliant unit environment; respond to call bells and emergencies; participate in occasional floating to other units as needed.
- Ensure effective handoffs and collaboration with the interdisciplinary team to support transitions from hospital to post-acute care.

Requirements
- Active LPN license in Vermont
- BLS certification
- EPIC experience within the past 2 years (required)
- Minimum 1 year of clinical experience; post-acute/long-term care experience preferred
- Prior travel experience required
- IV certification preferred; dementia-capable care certification preferred
- Ability to work a 12-hour night shift and to float to other units as needed; able to work all holidays
