Responsibilities
- Conduct occupational therapy evaluations and develop individualized treatment plans to improve activities of daily living, mobility, balance, and safety
- Implement evidence-based OT interventions and adapt plans based on resident progress and regulatory requirements
- Document progress and outcomes in the medical record, ensuring accuracy and confidentiality
- Collaborate with an interdisciplinary team, including nursing and rehab colleagues, to align therapy goals with overall care plans
- Educate residents and families on home exercise programs, adaptive equipment, and discharge planning
- Monitor progress, adjust treatment approaches, and participate in discharge planning and transition to next care setting
Requirements
- Active occupational therapy license (state of practice)
- Graduation from an accredited OT program (Master’s or Doctorate)
- Ability to work in a skilled nursing facility setting
- Prior experience in rehabilitation or long-term care settings preferred
- Willingness to participate in a travel assignment in Waterville, ME
