Responsibilities
- Coordinate referral intake, clinical triage, and care coordination for patients on the inpatient Structural Heart Unit
- Review and triage referrals; coordinate records, diagnostics, and follow-up care
- Prepare cases for multidisciplinary review; educate patients and communicate next steps
- Perform administrative duties related to referral management and care coordination; document activity in Epic; manage referral follow-ups
- Support care coordination efforts by tracking patient progression through the evaluation process and coordinating with other teams as needed
Requirements
- Active RN license in Massachusetts
- 2+ years RN experience; triage experience required; Level 1 trauma experience required
- Epic experience required; Excel experience required
- BLS (AHA) certification required
- Ability to function in an administrative, non-patient-facing care coordination role within an inpatient unit; orientation is weekly on Mondays
- Non-local travel status: must reside outside a 50-mile radius of the facility
