Responsibilities
- Conduct OT evaluations and develop individualized treatment plans to improve independence in activities of daily living and mobility for residents.
- Implement and monitor evidence-based interventions addressing ADLs, mobility, fine motor skills, cognition, and safety; adjust as progress is made.
- Reassess residents regularly, document outcomes, and update care plans accordingly in the electronic health record.
- Collaborate with an interdisciplinary team to coordinate care, attend care planning meetings, and provide education to staff and family.
- Facilitate discharge planning and refer for equipment or community services as needed.
- Participate in quality improvement initiatives and maintain infection control and regulatory compliance in daily practice.

Requirements
- Active Massachusetts Occupational Therapist license or eligibility for licensure
- Master's or Doctorate degree in Occupational Therapy from an accredited program
- 1+ year of OT experience in geriatric or skilled nursing settings preferred
- NBCOT certification preferred
- Excellent communication and documentation skills; ability to collaborate with interdisciplinary team
- Ability to work in a skilled nursing facility environment and commit to a 13-week travel assignment starting 11/16/2026
