Responsibilities
- Conduct biopsychosocial assessments and coordinate discharge planning for adult and geriatric patients
- Develop and implement care coordination plans to facilitate safe and timely transitions across care settings
- Collaborate with interdisciplinary teams (PT, Pharmacy, Transportation, Interpretation, Social Services) to optimize patient outcomes
- Communicate with patients, families, and internal/external providers regarding care plans, resources, and next steps
- Navigate Medicare/Medicaid processes and support high-risk interventions as appropriate
- Document thoroughly in the electronic charting system and participate in rounds as part of the care team

Requirements
- Active Washington clinical license in-hand (LMSW or LCSW) and BLS certification
- Master’s degree in Social Work (MSW) from an accredited program; LMSW or LCSW accepted
- Minimum 1 year of clinical experience in acute care, discharge planning, or care coordination (note: internal screening may require 2 years for submittal)
- EPIC charting experience; ability to complete required documentation promptly
- Ability to float across hospital areas and to related campus locations within a short distance; weekend requirement every other weekend
- Not accepting local candidates; 3-month minimum return-staff rule applies before reassigning as traveler
- Orientation and pre-start modules completed prior to start; 40 hours of orientation
