Responsibilities
- Conduct nutrition assessments and develop clinically appropriate care plans for patients within a hospice/home health setting
- Provide medical nutrition therapy, monitor patient response, and adjust plans as needed to support palliative goals
- Collaborate with an interdisciplinary team to integrate nutrition services into overall patient care
- Document nutrition care and progress in the designated electronic documentation systems
- Educate patients and family members on diet, hydration, and oral intake considerations aligned with end‑of‑life goals
- Participate in routine care planning and advance care discussions as part of the care team
Requirements
- Clinical nutrition experience, particularly in hospice or home health settings
- Proficiency with electronic documentation systems (Cerner and CBORD) as noted in assignment details
- Ability to work daytime, 8-hour shifts (40 hours/week) as part of a 5x8 schedule
- Willingness to participate in a 16-week travel assignment in the Lodi, CA area
- Strong collaboration and communication skills for interdisciplinary patient care
