Travel Physical Therapist General Job
Physical Therapist (General) – Albuquerque, NM. Travel assignment starting 08/31/2026 for 13 weeks in a home health care setting. Earn $2,369–$2,492 per week with a guaranteed 30 hours per week. Shift is Mon–Fri. Home health patients will be seen within a 45-mile radius of the Albuquerque area.

Responsibilities
- Provide physical therapy services within the legal scope of practice for the state, maintaining licensure and required certifications; perform initial evaluations, progress notes, 30-day reassessments, and 60-day recertifications in line with Medicare, state, and CareM standards.
- Develop, implement, and update individualized plans of care aligned with physician orders and patient needs; deliver skilled interventions and maintenance therapy programs as permitted by state scope.
- Directly supervise Physical Therapist Assistants (PTAs) and Home Health Aides in compliance with state practice acts; consult with physicians and interdisciplinary team members regarding treatment adjustments and patient progress.
- Educate patients and caregivers on home exercise programs, mobility, and adaptive strategies to promote independence and safety; prepare and submit OASIS assessments, recertifications, transfers, resumptions of care, and discharges within agency timeframes.
- Apply ICD-10 coding accurately for compliant documentation; present in-service programs and contribute to ongoing staff education; actively engage in Quality Assessment and Performance Improvement (QAPI) activities.
- Document interventions and care coordination timely; participate in interdisciplinary meetings, case reviews, and continuing education; adhere to HIPAA and agency privacy standards; work independently with initiative in home settings and demonstrate professional communication with patients, families, and team members.
Requirements
- Current Physical Therapist license in the state of practice (New Mexico) or equivalent as required.
- Graduate of an accredited physical therapy curriculum approved by the CAPTE, APTA, or similar accrediting body.
- Two years of clinical experience preferred; home health experience is a plus but not required.
- CPR certification active; ability to exercise sound clinical judgment and work independently in varied patient care settings.
- Basic computer proficiency; willingness to learn EMR systems and OASIS documentation.
- Valid driver’s license with reliable transportation for home visits; maintain compliance with state regulations and agency policies.
