Responsibilities
- Provide direct patient care to residents within the scope of LPN practice in a long-term care setting
- Monitor and document vital signs, administer medications per physician orders, and assist with treatments as assigned
- Support activities of daily living and mobility needs for residents in collaboration with the care team
- Observe changes in resident condition and report promptly to the supervising RN or charge nurse
- Maintain accurate, timely charting and documentation according to facility policy and regulatory requirements
- Ensure safety, infection control, and quality standards; uphold resident dignity and confidentiality
Requirements
- Licensed Practical Nurse (LPN) license; active and in good standing
- Ability to work night shifts (18:00–06:30) and meet assignment scheduling needs
- Minimum competency in clinical skills typical for long-term care settings, including medication administration and patient monitoring
- Strong communication and teamwork skills to collaborate with nursing staff and interdisciplinary colleagues
- Adaptability to a fast-paced travel assignment and varied resident needs
