Responsibilities
- Plan, organize, and direct home care services to meet the needs of patients in their homes, aligning care with the patient’s plan and goals.
- Manage a patient caseload in an efficient and effective manner, prioritizing needs and coordinating care across disciplines.
- Provide patient care independently while collaborating with core disciplines (nursing, social work, rehabilitation, and other team members) to meet care goals.
- Serve as a resource to patients, family members, and other care team members; provide education, support, and guidance to promote safety and adherence to the care plan.
- Document care plans, progress, and outcomes in the patient’s record; coordinate communications with physicians and other providers; ensure compliance with standards and regulations.
Requirements
- Active, unrestricted Registered Nurse license in California
- Experience in case management and/or home health care
- Ability to manage a patient caseload and work independently, with strong organizational skills
- Excellent communication and collaboration skills for working with patients, families, and multidisciplinary teams
- Ability to travel to patient homes as required; reliable transportation
- Proficiency with electronic medical records and routine documentation
