As part of the LTC RN role, you will lead comprehensive assessments for new admissions, monitor chronic conditions, administer medications with precision, manage wounds, implement fall-prevention strategies, and collaborate with physicians, therapists, social workers, and families to optimize recovery and quality of life. You’ll coordinate individualized care plans, document progress, and adjust interventions based on evolving conditions, always prioritizing safety, comfort, and dignity.
