Responsibilities
- Deliver coordinated nursing care for a patient or assigned group of patients according to established standards of care and the nursing process.
- Assess patient condition, monitor vital signs, administer medications and therapies as prescribed, and document care accurately.
- Develop, implement, and evaluate care plans in collaboration with the interdisciplinary team; adjust plans as patient needs evolve.
- Supervise and direct activities of various levels of assigned nursing staff; provide supervision with professional judgment.
- Coordinate care with other disciplines, communicate effectively with patients and families, and exercise critical thinking and independent judgment.
- Ensure adherence to policies, regulations, and quality standards; maintain accurate charting and compliance with documentation requirements.
Requirements
- Active Registered Nurse license in California
- Ability to work in a home healthcare hospice setting
