Responsibilities
- Administer prescribed medications and treatments according to orders and facility policies.
- Monitor vital signs, assess patient status, and communicate changes to the supervising nurse.
- Provide skilled nursing tasks and activities of daily living support under supervision.
- Assist with care plan implementation and collaborate with the interdisciplinary team.
- Support wound care, infection control, safety protocols, and falls prevention.
- Document care in the charting system and maintain patient privacy and confidentiality.
Requirements
- Active LPN license in Pennsylvania.
- Graduate of an approved practical nursing program.
- Ability to work in a fast-paced long-term care environment; strong communication and teamwork skills.
- Prior LTC experience preferred.
- Proficiency with basic computer documentation and regulatory compliance.
