Responsibilities
- Provide direct patient care and assessment for ICU-level patients in a home hospice setting, including monitoring, documentation, and timely escalation as needed.
- Administer prescribed medications and IV therapies per physician orders; manage symptoms and establish comfort goals in collaboration with the care team.
- Monitor vital signs and clinical status; respond to changes with appropriate interventions and communicate updates to the physician.
- Collaborate with an interdisciplinary team to develop and implement individualized care plans and goals of care for patients and families.
- Educate patients and family caregivers on disease processes, treatment plans, medication administration, safety, and caregiver support in the home environment.
- Uphold infection control, safety, and quality standards; maintain accurate records and ensure compliance with regulatory requirements.
Requirements
- Active RN license in California
- ICU experience: minimum 3 years; at least 3 months recent ICU experience in the specialty
- AHA BLS and AHA ACLS required
- Hospital fire card required
- Acute care facility experience preferred
- BSN preferred
- Ability to work in a home healthcare - hospice setting, including nights
- When submitting, include DOB, SSN, preferred start date, and any RTO
