Registered Nurse - Case Manager (Travel) in Everett, WA, United States. Specialty: Case Manager. Start date: 05/04/2026. Duration: Weeks (Travel - Temporary Assignment). Responsibilities
- Perform discharge planning and care coordination for patients across Med-Surg, long LOS, and ED populations; develop and implement discharge plans and transition of care.
- Conduct needs assessments, risk stratification, and plan of care development; coordinate post-acute services, DME, and appropriate follow-up.
- Collaborate with the Interdisciplinary Team, including Social Services, to ensure safe transitions and CMS/compliance requirements are met.
- Communicate with patients and families about care plans, follow-up appointments, medications, and available community resources; provide education as needed.
- Document plans and progress in EPIC; maintain accurate, timely, and compliant records.
- Manage a caseload with a typical patient load of 25–30, and float across hospital areas as assigned to support patient throughput and continuity of care.
Requirements
- Active Registered Nurse license in Washington; pending licensure accepted for submission
- BLS certification
- Minimum 1 year of acute care experience with discharge planning / care coordination
- Experience in case management across Med-Surg, long LOS, and ED preferred
- EPIC charting experience; ability to work across hospital units and adapt to CMS requirements
- This is a travel assignment; locals are not accepted; first-timers welcomed; pending license accepted for submission
