Responsibilities
- Conduct initial and ongoing speech-language pathology assessments for residents in the LTC setting
- Develop, implement, and monitor individualized treatment plans to address communication and swallowing disorders
- Provide direct therapy services to residents, including progress monitoring and goal modification
- Document progress notes, treatment plans, and outcomes in the resident’s chart
- Collaborate with interdisciplinary care team to coordinate care and discharge planning
- Educate residents and families on therapy goals, expectations, and home strategies
Requirements
- Active state licensure as a Speech-Language Pathologist in the United States or eligibility to obtain licensure in Wyoming
- Ability to work effectively in a long-term care setting
- Strong written and verbal communication skills; proficient documentation
- Flexibility to travel for a 13-week assignment and work daytime hours
- Master’s degree in Speech-Language Pathology from an accredited program
- Comfortable using standard charting and documentation systems
