Responsibilities
- Lead case management, discharge planning, and care coordination for an acute care patient population from admission to discharge
- Perform comprehensive needs assessments, identify discharge barriers, coordinate DME and post-acute services, and implement the plan of care with the interdisciplinary team
- Collaborate with Social Services, nursing staff, physicians, and other care providers; participate in rounds and care conferences; coordinate transitions of care across services
- Maintain accurate EPIC documentation, updating the plan of care, risk assessments, and discharge notes in a timely manner
- Manage a caseload of approximately 25 patients, with flexibility to float across hospital areas and to sister campuses as needed

Requirements
- Licensure: Registered Nurse licensure in Washington; pending license accepted, or an active MSW credential with relevant case management experience in acute care
- Certifications: Basic Life Support (BLS) required
- Experience: Minimum 1 year of acute care experience with discharge planning / care coordination; first-time travelers accepted
- Skills: EPIC charting experience (required); strong assessment, coordination, and communication abilities
- Setting: Ability to work effectively in an acute hospital setting and with a broad age range from neonates to geriatrics
- Education: RN degree from an accredited program or MSW degree applicable to case management
