Responsibilities
- Administer medications and treatments per physician orders under supervision
- Monitor residents’ conditions, document vital signs and changes
- Perform nursing assessments, develop and adjust care plans in collaboration with the interdisciplinary team
- Provide direct patient care and assistance with activities of daily living
- Assist with routine procedures, wound care, and infection prevention as directed
- Communicate changes and outcomes to the supervising registered nurse and maintain accurate documentation
Requirements
- Active LPN license with ability to practice in Vermont
- Experience in long-term care or geriatric settings preferred
- Strong assessment, documentation, and communication skills
- Ability to work evenings and/or overnight shifts
- Demonstrated reliability and ability to collaborate within a care team
