Responsibilities
- Conduct initial physical therapy evaluations and develop comprehensive treatment plans for residents in a long-term care setting.
- Deliver therapy to address mobility, strength, balance, gait, and functional independence goals.
- Reassess residents regularly and update goals and treatment plans based on progress.
- Document assessments, treatment sessions, and progress notes in the appropriate medical record per regulatory standards.
- Collaborate with an interdisciplinary team to coordinate care, discharge planning, and transition of services.
- Educate residents and families on exercises, safety, and home-program plans to support ongoing progress.
Requirements
- Current, unrestricted physical therapy license to practice in the United States
- Doctor of Physical Therapy (DPT) degree or equivalent
- Experience in a skilled nursing facility or long-term care setting preferred
- Strong assessment, documentation, and communication skills
- Ability to work the stated shift and meet guaranteed hours in a travel assignment
