Responsibilities
- Collect data on residents’ physical and psychosocial conditions and communicate findings to the RN and Interdisciplinary Team.
- Contribute to a developmentally appropriate and culturally relevant plan of care reflecting nursing diagnoses and interventions for adult and geriatric residents, involving residents, families, and the healthcare team.
- Recommend revisions to the plan of care after evaluating resident outcomes and coordinate referrals to ensure all aspects of care are addressed.
- Collaborate with residents/families and Interdisciplinary Team within the scope of nursing practice to promote a safe, satisfactory care environment.
- Under the supervision of an RN, provide direct and indirect skilled nursing care, maintain current knowledge, participate in mentoring or orientation of staff, and demonstrate leadership and clinical competency.
Requirements
- Active NY LPN licensure
- BLS certification (American Heart Association or RQI program)
- Minimum 1 year of LPN experience in a long-term care setting
- Ability to provide care for adult and geriatric residents
- Availability to start orientation on a Tuesday or Thursday for full-shift onboarding
- Willingness to work a rotating shift in a travel assignment
