Responsibilities
- Conduct initial evaluations and develop individualized treatment plans for adult patients requiring physical therapy in a SNF setting.
- Implement and progress evidence-based therapy programs emphasizing mobility, gait, strength, balance, transfers, and functional independence.
- Document patient assessments, treatment sessions, and progress notes; communicate updates to the care team and adjust plans as needed.
- Provide patient and caregiver education on exercises, safety, and discharge planning; collaborate with the interdisciplinary team for smooth transitions.
- Ensure patient safety, proper use of modalities and equipment, and compliance with regulatory and infection control standards.

- Strong communication and documentation skills
- Experience or comfort providing geriatric rehabilitation services
- Ability to complete onboarding and credentialing processes as part of travel assignments
