Responsibilities
- Conduct comprehensive PT evaluations and develop individualized treatment plans for residents.
- Implement therapy interventions including gait training, therapeutic exercises, range of motion, strengthening, balance training, and functional mobility activities.
- Monitor patient progress, reassess regularly, and adjust plans of care accordingly.
- Maintain accurate and timely documentation; communicate progress and needs with the interdisciplinary care team.
- Collaborate with nurses, physicians, and support staff to support discharge planning and transition of care.
- Educate residents and families on therapy goals, home programs, and safety strategies to maximize independence.
Requirements
- Active Physical Therapist license in good standing.
- Ability to work in a skilled nursing facility setting with geriatric patients.
- Strong documentation and communication skills; ability to collaborate within an interdisciplinary team.
- Availability for a 13-week assignment with guaranteed hours and a day shift schedule.
- Willingness to travel to and work at the assignment location as described.
