Responsibilities
- Administer prescribed medications and treatments to residents according to orders, monitor for adverse reactions, and document administration in the chart.
- Conduct regular nursing assessments, monitor vital signs and pain levels, and report findings to the supervising RN for care plan updates.
- Implement and reassess individualized care plans under RN supervision, including assistance with activities of daily living and routine treatments.
- Assist residents with ADLs and mobility while prioritizing safety, dignity, and comfort.
- Collaborate with physicians, therapists, and other members of the interdisciplinary team to ensure continuity of care and timely communication.
- Recognize and escalate changes in resident condition promptly per facility policies and procedures.
Requirements
- Active LPN license in New Mexico (in good standing)
- Current Basic Life Support (BLS/CPR) certification
- Minimum 1 year of recent LPN experience in a long-term care or skilled nursing setting
- Graduate of an approved practical nursing program
- Ability to work 12-hour shifts in a long-term care environment
