Responsibilities
- Administer prescribed medications and treatments per physician orders, documenting administration accurately.
- Monitor residents’ health status, perform ongoing assessments, and record vital signs and changes.
- Provide routine nursing care and assist with activities of daily living to support resident independence and comfort.
- Collaborate with RNs and the interdisciplinary team to update care plans and communicate changes.
- Maintain accurate documentation in the electronic health record and ensure clear handoffs during shift changes.
- Support safety and patient comfort, including wound care and standard nursing procedures as trained.
Requirements
- Active LPN license in good standing (state of practice)
- Ability to work in a skilled nursing/long-term care environment
- Strong communication and teamwork skills
- Proficiency with electronic health records and charting
- Willingness to work a 3x12-hour shift pattern
- Availability for a travel assignment in Clovis, NM
