This travel RN role provides skilled nursing care for patients in their homes under a home health model. The assignment supports coordination with the interdisciplinary team and caregiver networks to deliver high-quality, patient-centered care.

Responsibilities
- Assess home health and hospice patients in stable and emergent situations, identifying physical, psychosocial, and environmental needs; document findings and coordinate care with the interdisciplinary team.
- Develop, implement, document, and evaluate the Plan of Care to ensure quality and continuity of care; update as patient needs change.
- Obtain data on physical, psychological, social, spiritual, and cultural factors and incorporate findings into the Plan of Care.
- Maintain up-to-date patient records; ensure visits, contacts, problems, plans, actions, and goals are accurately documented and reflected as changes occur.
- Provide care using infection control measures; educate patients and families on care, medications, home safety, and self-care practices; tailor teaching to age, condition, culture, and learning readiness.
- Supervise and provide clinical direction to home health aides and other staff as assigned; participate in interdisciplinary team meetings and coordinate referrals when needed.

Requirements
- Active RN license in Tennessee (or current state licensure eligible for practice in TN)
- OASIS experience required
- Homecare Homebase software experience required
- 2+ years of home health nursing experience; 1-2 years RN clinical care setting experience
- CPR certification required; valid driver’s license with reliable transportation and insurance
- ADN or Nursing Diploma required; BSN preferred
