Responsibilities
- Provide comprehensive RN care to hospice/home health patients in home settings, focusing on symptom management and comfort.
- Conduct patient assessments, monitor vital signs, manage medications, and adjust care plans in collaboration with the multidisciplinary team.
- Administer prescribed therapies and monitor for adverse reactions, ensuring safety and quality of care.
- Document all clinical activities and updates in the Cerner FirstNet EMR with accuracy and timeliness.
- Educate patients and families on disease progression, treatment options, medications, and end-of-life care; offer emotional and caregiver support.
- Coordinate with physicians, social workers, and other team members to ensure continuous, patient-centered care and appropriate resource use.
Requirements
- Active California RN license
- Ability to work night shifts (12-hour) and participate in weekend coverage as needed to support 24/7 patient care
- Experience in or ability to work in a home healthcare or hospice environment
- Proficiency with Cerner FirstNet EMR or willingness to learn the system
- Strong communication and interpersonal skills; ability to work independently in patient homes and adapt to varied environments
