Responsibilities
- Plan and conduct segments of treatment programs designed to restore and improve physical function, while aligning with department objectives.
- Perform evaluations and ongoing assessments to monitor progress and refine individualized treatment plans.
- Deliver therapy interventions using evidence-based protocols and adapt techniques to meet patient goals.
- Collaborate with an interdisciplinary team, and co-lead patient groups as appropriate.
- Complete comprehensive clinical documentation within established deadlines and communicate discharge plans to patients, families, and care partners.
- Supervise assigned PT aides or assistants and ensure therapy quality and compliance with standards.
Requirements
- Hawaii state Physical Therapist license (active)
- 2 years of physical therapy experience (required)
- 1 year of physical therapy home health experience (required)
- Bachelor's degree in Physical Therapy; Master's degree preferred
- Homecare Home Base and OASIS experience required
- CPR and BLS (AHA) certifications
- Travel experience preferred; willingness to travel within the assignment area
- Ability to work effectively in a home health care setting and manage patient care in a community-based environment
