Responsibilities
- Administer prescribed medications and document administration in accordance with policy and state regulations
- Monitor residents’ health status, including vital signs, and report changes to the supervising RN
- Assist with activities of daily living (ADLs) and provide comfort measures to residents
- Maintain accurate nursing notes, care plans, and assessments in the electronic health record
- Collaborate with the interdisciplinary team to coordinate care, ensure safety, and support transfers or transitions as directed
- Adhere to infection control practices and safety protocols to protect residents and staff
Requirements
- Active LPN license in good standing
- Experience in long-term care or geriatric care preferred
- Ability to work evenings and/or overnight shifts
- Strong communication and teamwork skills
- Eligible to travel for assignment in Vermont or willing to relocate for the duration of the assignment
