Social Worker – Medical Social Work travel assignment in Everett, Washington. Start date 05/18/2026 for 12 weeks. Weekly pay range $1,683–$1,770. Shift is 5x8 Days. This short-term assignment takes place in a hospital setting with Level II Trauma Center coverage across two nearby campuses. Orientation is 40 hours and the role requires cross-campus coverage with weekend and holiday availability.

Responsibilities
- Deliver discharge planning and care coordination for adult and geriatric patients; identify barriers to discharge and develop care plans in collaboration with the interdisciplinary team
- Conduct psychosocial assessments, document plans of care, and coordinate referrals to home health, DME, rehab, or long-term care as appropriate
- Maintain an active caseload (typically 18–24 patients daily; weekend caseloads may exceed 30) and facilitate smooth transitions of care
- Collaborate with PT, Interpretation Services, Social Services, Pharmacy, Transportation, and other disciplines to support patient needs
- Document thoroughly in EPIC, update the plan of care, and ensure compliance with hospital policies and regulatory requirements
- Float across hospital areas as needed, including across two campuses, to support patient care and workload balance
Requirements
- Active Washington social work licensure or eligibility accepted per facility policy; credentials accepted include LCSW, LICSW, LSWAIC; non-clinical credentials LASW, LSWAA
- BLS certification; EPIC charting experience required
- Minimum 1 year of acute care experience with discharge planning / care coordination; diverse case management experience (Med-Surg, LLOS, ED) preferred
- Ability to work across multiple hospital areas; weekend and holiday coverage required
- Local candidates not accepted for this assignment
