- Setting: Acute care and clinic environments (short term acute care hospital and outpatient)
Responsibilities
- Evaluate and treat patients in acute care and nursing home settings; screen patients and attend team meetings to discuss progress.
- Document evaluations, progress notes, daily notes, insurance documentation, and discharge notes in the patient’s medical records.
- Collaborate with interdisciplinary team members to ensure effective carryover of services and seamless patient care transitions.
- Provide outpatient evaluation and treatment when indicated.
- Perform additional duties as assigned to support patient outcomes and program goals.
Requirements
- Education: Master of Occupational Therapy required; Doctorate preferred.
- Licensure/Certification: National Board Certification in Occupational Therapy; Wisconsin occupational therapy license; continuing education to maintain licensure; BLS certification required.
- Experience: 1+ year of related work experience.
- Credentials: Valid driver’s license; ability to travel to assigned sites as needed.
- Competence: Exceptional accuracy and attention to detail; ability to work in acute care and nursing home settings; strong teamwork and communication skills.
