Responsibilities
- Deliver direct patient care under the supervision of the nursing team in a hospital setting, focusing on comfort, safety, and functional support for patients in long-term care scenarios
- Administer prescribed medications and therapies according to treatment plans and physician orders
- Monitor and document vital signs, patient status changes, and responses to interventions; report significant findings promptly
- Assist patients with activities of daily living, mobility, and daily routines; promote fall prevention and safety
- Maintain accurate, timely nursing documentation; participate in care planning and multidisciplinary rounds as appropriate
- Adhere to infection control practices, medication safety protocols, and patient privacy standards; ensure proper device and equipment management
- Collaborate with licensed professionals to coordinate care, communicate changes, and ensure continuity of treatment across shifts

Requirements
- Active Licensed Practical Nurse license in New Hampshire
- BLS (American Heart Association) certification required
- Minimum 2 years of current LPN experience
- LPN skills checklist completed and submitted
- Supervisory signed reference from the past year of employment (required)
- License verification documentation (required)
- All licenses and certifications current at time of submission
- Permanent address included in submission file
- RTO (requested time off) included in submission
