Responsibilities
- Deliver direct ER-level nursing care to hospice patients in a home setting during night shifts, including ongoing assessment, triage, stabilization, and symptom management per physician orders.
- Administer prescribed medications, monitor patient status, and respond to evolving clinical needs consistent with hospice and emergency protocols.
- Document assessments, interventions, and patient responses in the electronic health record; communicate changes to the broader care team.
- Collaborate with physicians, case managers, and home health staff to adjust care plans, ensure safety, and coordinate timely escalation when needed.
- Educate patients and families on treatment plans, safety precautions, and end-of-life care considerations; uphold infection control, safety standards, and fire and life safety requirements.
Requirements
- Active California RN license (ER/acute care) required
- ER nursing experience required
- BLS and ACLS certifications required
- LA Fire and Life Safety card required
- Cerner and Epic experience preferred
- Ability to work night shifts in a home-based hospice setting
