Responsibilities
- Deliver direct patient care to residents per plan of care under supervision of the RN
- Administer prescribed medications and document administration accurately in the chart
- Monitor vital signs, assess for changes, and report promptly to the nursing team
- Assist residents with activities of daily living and mobility; promote comfort and safety
- Support care coordination with the interdisciplinary team and update care plans as needed
- Maintain accurate and timely documentation in the electronic health record
Requirements
- Active LPN license (state of New Mexico) or eligibility to practice
- Basic Life Support (BLS) certification
- 1 year of recent LPN experience in a skilled nursing or long-term care setting preferred
- Ability to work a night shift schedule (3x12-hour shifts)
- Diploma or certificate from an approved LPN program
- Willingness to meet onboarding and credentialing requirements as part of the assignment
