Responsibilities
- Deliver coordinated nursing care for a patient or assigned group of patients according to established standards of care and the nursing process.
- Supervise and direct the activities of various levels of assigned nursing staff.
- Coordinate care with other disciplines while utilizing critical thinking, professional and supervisory discretion, and independent judgment.
- Provide symptom management, safety, and comfort in the home hospice setting and document care in patient records.
- Collaborate with an interdisciplinary team to implement and adjust care plans as needed.
Requirements
- Ability to work 12-hour night shifts (36 hours per week)
- Experience delivering nursing care in a home healthcare or hospice setting
- Strong critical thinking and independent judgment
- Supervisory or leadership experience coordinating care across disciplines
