Responsibilities
- Provide direct and indirect skilled nursing care to residents under the supervision of a Registered Nurse.
- Collect data on residents’ physical and psychosocial status and communicate findings to the RN and Interdisciplinary Team.
- Contribute to development and ongoing modification of an age-appropriate, culturally relevant plan of care, incorporating resident/family input and nursing diagnoses.
- Administer prescribed medications and treatments within scope, monitor responses, and document outcomes; escalate changes as needed.
- Maintain safety and infection control practices and support a safe, clean environment for geriatric residents.
- Collaborate with physicians, nurses, and other team members; assist with staff mentoring/orientation and participate in continuing education as requested.
Requirements
- Current New York LPN license (active)
- BLS certification (AHA or RQI accepted)
- Minimum 1 year of LPN experience in a long-term care setting
- Ability to work night shifts and every other weekend per assignment (Fri & Sat nights)
- LPN diploma or equivalent education for eligibility
