Responsibilities
- Deliver direct nursing care to residents on the post-acute/long-term care unit, adhering to care plans and policies.
- Monitor patient status, record vital signs, assessments, and progress notes; escalate changes per protocol.
- Administer medications and treatments as ordered, using the EMR (EPIC) for documentation.
- Collaborate with the interdisciplinary team to develop, implement, and update resident care plans.
- Assist residents with activities of daily living and mobility; provide safe, respectful patient-centered care.
- Respond to call lights and emergencies; ensure safety and comfort of residents; participate in rounds and discharge planning as needed.
- Float as required to meet unit needs; maintain competency across assigned areas.
Requirements
- Current Licensed Practical Nurse license in Vermont (LPN)
- Minimum 1 year of practical nursing experience
- Prior travel nursing experience required
- EPIC EMR experience within the past 2 years is required
- Dementia-capable care certification preferred
- BLS certification required
- Ability to work Night shifts; willingness to float as needed
- Reliable transportation is required (personal vehicle or rental strongly recommended)
