Responsibilities
- Provide direct nursing care to adult residents on the post-acute unit, including assistance with activities of daily living and basic medical procedures, per physician orders.
- Monitor patient conditions, perform ongoing assessments, and promptly report changes to the interdisciplinary team; respond to emergencies and call lights as needed.
- Administer medications and treatments in accordance with facility policies, maintaining accurate documentation and appropriate safety checks.
- Document care and clinical information promptly in the EPIC EMR; ensure data is complete and up-to-date within the past two years of EPIC experience.
- Uphold dementia-capable care practices, promote patient safety, and support residents’ comfort and dignity during night shifts.
- Float to other units as required to meet patient care needs and unit staffing, while maintaining consistent standards of care.
Requirements
- Active Vermont Licensed Practical Nurse (LPN) license
- Minimum 1 year of nursing experience; experience in long-term care or post-acute settings preferred
- EPIC EMR experience within the past 2 years
- Prior travel nursing experience
- BLS certification required
- Dementia-capable care certification preferred
