Responsibilities
- Provide nursing care for assigned patients under the supervision of a registered nurse, following established policies and the nursing process
- Perform routine visits and clinical procedures in accordance with department policy
- Administer prescribed medications and support treatment plans
- Assist in development and execution of interdisciplinary care plans; report patient progress and changes
- Support patient and family education, discharge planning, and transitions of care
- Restock and organize supplies; maintain cleanliness and safety in assigned areas
- Participate in on-call duties as required by agency operations

Requirements
- Licensure: Licensed Vocational Nurse (LVN) license in the state of practice (California) required
- Certifications: Basic Life Support (BLS) Health Care Provider certification required; arrhythmia certification preferred
- Experience: At least two years of recent nursing experience in the field
- Education: High School diploma or GED required; bachelor’s degree preferred
- Skills: EPIC electronic medical record experience and hospice care experience preferred; ability to work in a hospital/acute care setting
- Additional: Weekend availability and ability to work 3 days during the week; valid driver’s license preferred
