Registered Nurse – Home Health (Travel) in Denver, Colorado, United States. Start date: 04/20/2026. 14-week travel assignment with 40 guaranteed hours per week on an 8-hour shift. Responsibilities
- Assess home health and hospice patients in stable and emergent situations to identify physical, psychosocial, and environmental needs; document findings for the care team.
- Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
- Gather data on physical, psychological, social, spiritual, and cultural factors and incorporate them into the POC.
- Maintain up-to-date patient records with visits, problems, plans, actions, goals, and changes reflected promptly.
- Educate patients and families regarding care, medications, home safety, diets, and self-care practices; tailor teaching to patient age, condition, culture, and readiness.
- Supervise and provide clinical direction to HHA/STNA/LVN staff to ensure quality and continuity of service; participate in interdisciplinary team meetings as appropriate.
- Provide on-call coverage and respond to patient/family needs as assigned.
- Coordinate referrals to appropriate agencies and monitor care to meet third-party payer requirements.
Requirements
- Active Colorado Registered Nurse license – Required
- CPR certification – Required
- OASIS experience – Required
- Homecare Homebase software experience – Required
- 1-2 years RN clinical care experience – Required
- 2+ years home health experience – Required
- Education: ADN or Nursing Diploma from an accredited program – Required; BSN – Preferred
- CHPN certification – Preferred
- Current driver’s license and reliable transportation – Required
