Responsibilities
- Provide direct, resident-centered nursing care to adults in a long-term care setting
- Monitor vital signs, administer prescribed medications, and document care in the electronic medical record
- Assess resident status and promptly report changes to the supervising RN or physician; participate in care planning
- Assist with activities of daily living (ADLs) and restorative/rehabilitative activities as directed
- Maintain infection control practices, medication safety, and patient safety; respond to emergencies per facility protocol
- Collaborate with the interdisciplinary team to support individualized care plans and resident well-being
Requirements
- Active Licensed Practical Nurse (LPN) license in Massachusetts
- Graduation from an accredited practical nursing program (LPN diploma)
- Current Basic Life Support (BLS) certification
- Minimum 1 year of practical nursing experience in long-term care or skilled nursing settings (preferred)
- Ability to work nights (23:00-07:00) and adapt to the listed shift
- Strong communication, documentation, and teamwork skills; comfort working in a fast-paced LTC environment
