Physical Therapist — Outpatient, Mattoon, Illinois. This travel, temporary assignment begins 08/03/2026 for 13 weeks with guaranteed 40 hours per week. Weekly pay range: $2,225–$2,267. Shift is 8:00 AM–4:30 PM (5x8 days). The role supports outpatient rehabilitation services within a healthcare facility, focusing on patient-centered care to restore function and independence.

Responsibilities
- Plan and implement rehabilitation programs using therapeutic exercise, gait training, manual techniques, neuromuscular re-education, and modalities.
- Evaluate mobility, strength, and current function; set short- and long-term goals aligned with the Plan of Care.
- Develop and implement a Plan of Care, including a home exercise program and discharge instructions.
- Document patient progress per approved medical and facility procedures; ensure compliance with third-party payer requirements.
- Supervise and collaborate with therapy assistants and other facility staff to maintain an effective rehabilitation environment.
- Perform initial and ongoing physical therapy evaluations in accordance with APTA standards and state requirements; maintain professional knowledge through ongoing education.
- Communicate openly with patients, families, and referral sources; support continuity of care across the care team.
Requirements
- Active Illinois Physical Therapist License
- Active American Heart Association BLS (Basic Life Support)
- Minimum 1 year of physical therapy experience (Outpatient experience strongly preferred)
- Outpatient experience required
- Must reside more than 50 miles from Mattoon, IL (local candidates within 50 miles are not considered)
- Two Supervisory Reference Forms completed with dates of employment listed
- Ability to perform work in an outpatient rehabilitation setting and meet credentialing requirements
- Documentation of required education and credentials as requested
