Responsibilities
- Administer medications and provide routine nursing care to residents in accordance with care plans and physician orders
- Monitor resident status, document observations, and report changes to the supervising RN
- Perform basic nursing assessments and participate in care planning with the interdisciplinary team
- Support activities of daily living (ADLs) including mobility, hygiene, nutrition, and comfort measures
- Maintain accurate documentation in the electronic medical record and ensure proper charting of interventions
- Assist with wound care, infection control practices, and patient safety protocols as assigned

Requirements
- Active Licensed Practical Nurse (LPN) license in Vermont or eligible to practice in Vermont
- Ability to work day or evening shifts as scheduled
- Experience in a long-term care or skilled nursing setting preferred
- Strong communication and teamwork skills with residents, families, and care staff
- Eligible for a travel assignment and able to work in Barton, Vermont
