Responsibilities
- Administer prescribed medications and manage the medication administration record (MAR) according to physician orders
- Monitor residents’ health status, vital signs, and comfort; identify and report changes promptly
- Assist with activities of daily living and maintain resident safety, dignity, and quality of life
- Perform routine skilled nursing tasks and procedures under supervision and per facility policy
- Collaborate with the care team to update and implement individualized care plans
- Document assessments, interventions, and outcomes in the electronic health record, ensuring accuracy and timeliness
Requirements
- Active Licensed Practical Nurse license in Vermont (or eligibility to practice in Vermont)
- Completion of an accredited practical nursing program
- Current Basic Life Support (BLS) certification
- Experience in a long-term care or similar setting preferred
- Ability to work day or evening shifts and maintain a 40-hour work week
- Strong communication and teamwork skills; ability to document clearly and follow care plans
