Responsibilities
- Deliver direct patient care under the supervision of a registered nurse in a long-term care setting
- Administer prescribed medications and document administrations per policy
- Monitor and report changes in patient condition; track vital signs and activities of daily living
- Assist with daily care activities, including bathing, dressing, feeding, and mobility
- Maintain accurate, timely documentation in the patient record and ensure handoffs are clear
- Apply infection control practices and uphold patient safety and comfort during shifts
Requirements
- Active Licensed Practical Nurse license in New Hampshire
- CPR/BLS certification
- Minimum 1 year of licensed practical nursing experience; long-term care or skilled nursing experience preferred
- Ability to work the evening shift and adapt to a fast-paced environment
- Eligibility to work in the United States; successful background check and health screening; up-to-date immunizations as required
