Responsibilities
- Assess residents’ physical therapy needs and develop individualized treatment plans tailored to diagnosis, functional goals, and safety considerations.
- Implement evidence-based therapeutic interventions, including therapeutic exercises, gait training, balance activities, and functional mobility training.
- Monitor patient progress, document findings, and adjust treatment plans to achieve prescribed outcomes in collaboration with the care team.
- Educate residents and family members on exercise programs, safety strategies, fall prevention, and home exercise routines to support continued recovery after discharge.
- Participate in interdisciplinary rounds and coordinate care with nursing, physicians, occupational therapy, speech therapy, and social work to support discharge planning.
- Ensure adherence to applicable regulations, documentation standards, infection control practices, and patient safety guidelines.
Requirements
- Active New York license as a Physical Therapist
- Prior experience in a skilled nursing facility (SNF) required
- Basic Life Support (BLS) certification
- Ability to work a full-time 40-hour week in a fast-paced, resident-focused setting
- Strong communication and documentation skills; ability to collaborate effectively with a multidisciplinary team
- Availability to work Monday through Friday, with Saturday/Sunday coverage as required by assignment logistics
