Responsibilities
- Conduct comprehensive biopsychosocial assessments for patients and families in hospice care and document findings in Epic EMR.
- Provide psychosocial support, counseling, and care coordination; participate in interdisciplinary team meetings to align goals of care and discharge planning.
- Facilitate advance directives, goals-of-care discussions, and family meetings; connect patients and families with appropriate community resources and services.
- Assess safety, functional status, and psychosocial needs; assist with planning and coordination of post-acute or home-based support as needed; document lift requirements (up to 50 lbs intermittently) when applicable.
- Maintain HIPAA compliance and high standards of ethical practice; collaborate with lead social worker and other team members to optimize patient outcomes.
- Travel within the defined service area and maintain a reliable vehicle; ability to drive in varied weather and road conditions (gravel roads, snow, ice); hold an active driver’s license and BLS certification.
Requirements
- Education: Master’s in Social Work (MSW) required; actively pursuing licensure toward LCSW (Licensed Clinical Social Worker) in the appropriate jurisdiction.
- Experience: Minimum 2 years of clinical/ED/Medical Social Worker experience.
- Licenses & Certifications: Active driver’s license with reliable vehicle; active BLS certification.
- Skills: Proficiency with Epic EMR; ability to perform biopsychosocial assessments and documentation; comfort working with a population that is predominantly 65+.
- Other: Ability to work effectively in a hospice setting and collaborate with an interdisciplinary team; comfortable with lifting up to 50 lbs intermittently.
