Responsibilities
- Assess residents to establish PT goals and develop individualized treatment plans within scope of practice.
- Deliver evidence-based physical therapy interventions to improve mobility, strength, balance, gait, and functional independence.
- Reassess progress regularly and adjust the treatment plan; document outcomes in the clinical record.
- Collaborate with an interdisciplinary team to coordinate care, discharge planning, and progression to the next level of care as appropriate.
- Educate residents and families on home exercise programs, safety strategies, and ongoing maintenance.
- Maintain compliance with facility policies, regulatory requirements, and infection-control standards; ensure accurate and timely documentation.
Requirements
- Ability to work in a skilled nursing facility setting.
- Availability to start on 09/28/2026 and commit to a 13-week assignment.
- Capability to deliver care during 08:00–16:30 shifts across a 5x8 schedule.
- Strong documentation and communication skills for effective interdisciplinary collaboration.
- Willingness to adhere to facility policies and regulatory requirements; ability to travel for a temporary assignment.
