Responsibilities
- Provide comprehensive RN care for post-surgical patients in a home hospice context, including initial assessment, care planning, implementation, and ongoing evaluation of outcomes.
- Function as scrub technician when needed, preparing sterile instruments, maintaining the sterile field, and assisting the surgical team during procedures.
- Coordinate, manage, provide and delegate patient care; administer medications and therapies per physician orders; document changes and communicate with the care team.
- Monitor patient status, vitals, pain, wound healing, drains, and sedation needs; implement pain control strategies and infection prevention; maintain accurate documentation.
- Collaborate with physicians, therapists, social workers, and family caregivers to ensure a safe, coordinated transition from perioperative care to home-based support; provide patient and family education and resources.
Requirements
- Active California RN license
- CPR/BLS certification
- Minimum 1 year of perioperative, post-surgical, or home health/hospice nursing experience
- Education: Nursing degree from an accredited program (ASN or BSN)
- Ability to work in a home healthcare - hospice environment and perform scrub duties as needed
