Responsibilities
- Deliver coordinated nursing care for patients in the home hospice setting according to the nursing process and established standards.
- Collect relevant health data, analyze assessments, develop and implement a care plan, and evaluate progress toward outcomes.
- Collaborate with the interdisciplinary team, patients, and families; provide patient and family education; participate in discharge planning for continuity of care.
- Delegate appropriately and coordinate duties of the nursing team while maintaining a safe, healing environment.
- Document patient care in the electronic health record (Epic EMR) and communicate effectively with treating providers and care teams.
Requirements
- Active Registered Nurse license in the state of practice (California) or appropriate state license.
- Minimum two years of recent nursing experience in the field; acute care facility experience preferred.
- BSN degree preferred.
- BLS/CPR certification required; ACLS certification required.
- Hospital Fire and Life Safety (HLFS) training required.
- Epic EMR experience required.
- Ability to work in a home healthcare/hospice environment and adapt to patient- and family-centered care.
